Welcome to the New York State Well-Being & Life Survey

By participating, your household is doing a valuable public service by helping to guide public health policies and prevention programs across the state.

As noted in the letter your household received, the survey is designed to be completed by the adult (18+) who has the next (upcoming) birthday. The eligible adult who completes the survey will receive a $10 Amazon gift code immediately after completing and submitting the questionnaire as a thank you for their participation.
